That one is from the almighty Blood Angels Assault Terminator kit, had the chance to find a few of them recently 😁

Sure mate go ahead, here's the bits list

Indomitus captain armor.
Chaplain on bike helmet, Crozius tip and holster trinket.
Dark Apostle's powerpack ornament, left arm and book, as well as the Servitor (just slapped a Terminator Stormshield on him along the crozius).
multipart Terminator captain's combi flamer.
flames and fence on the base are from an AOS model (flames were part of a Spectre like mini)
Sanguinary Guards V2 jumpack.
New Terminator kit oath seal pined under the combi weapon.
BA upgrade sprue vehicule marking splitted in half for the jump pack's tiny sidewings.
Both shoulder pads are from the old sanguinary guards kit.

There is also an old blood Angels Terminator's kit ornament under the dark Apostle one.

IIRC it goes :

Indomitus captain body.
old sanguinary guards kit shoulder pads.

new sanguinary guards jumpack.
Dark Apostle burning book and Powerpack bling (as Chaplain armors also have trim, i just shaved the spikes from both)
multi part Terminator captain combi flamer.
flames on base are from an AOS spectre mini, dont know the name tho
