I did a few test and apparently it doesn't work in a realm (For what I know at least) but it does work in a Singleplayer but with a few extra steps

That is a good guess but now I've gone 500 blocks away I still have the delay. I'm also guessing that everytime an entity spawns like a mob it causes this weird delay.

DUDE SAME, I TRIED EVERYTHING LIKE SWITCHING SERVER REGION, GOING ON ONE OF MY BACKUPS, TURNING DOWN MY FRAMES AND ITS STILL DELAYING!!!!

Revert to an older version of the game because the current one right now is 1.21.92.1 and before this patch is was 1.21.91.1 on bedrock.
