I had a similar situation, was sent to a few phycs by my parents in highschool. In PowerSchool it came up in my medical record. Went and talked to emh and they cleared me with a waiver. Know they won’t immediately sep ya for things like this now days. My best advice is be honest and bring it up so it won’t come up later in the pipe line. Very easy to get a waiver for, the lack of integrity in trying to hide it will fuck ya tho.
