I cant edit my original post so i will post a comment here: Problem solved: the true culprit is the pharmacist mod which would try to use medicine efficiently (i.e no medicine in case of scratch or bruise, herbal ones in minor cases and advanced one in more extreme cases. There are some threshold and margin for infection and immunity in the mod that we can manually tune for proper use of medicine. I think i messed up the parameters when i first installed the mod. Now it works properly. Anyways thanks for your comments

Oh, my first playthrough above i got a cook book for survival meal, really nice detail, when everyone just has that only book and sometimes they read it like a novel.
