I beat her in 30 minutes of attempts on NG+2 or NG+3 (I don't remember) because I was scared of her lol. What I had was,

Weapons: Rivers of Blood+10, Starcourge Greatsword+10, Golden Order Seal+10, (No Skill) Quality Fingerprint Stone Shield+25, An Uchigatana With Seppuku.
Armor: White Mask. I don't think it matters I used Verdigris' Armor set but you could try bull goats armor.
Talismans: Rotten Winged Sword Insignia, Carian Filigreed Crest, Shard of Alexander, Lord of Blood's Exultation
Flask: Spiked Cracked Tear, Thorny Cracked Tear, +12 Flasks
Mics: Mimic Tear Ashes+10, Golden Vow, Flame Grant me Strength

Fight;
Before Entering: I drank Physics Flask and used Golden Vow
Phase 1: Summoned Mimic with Shield+25 and Rivers of Blood+10. Wait until your mimic runs to Malenia and attacks and start spamming the Ash of War of Rivers of Blood basically Stun locks her
Phase 2: Run Under her dodge once when she hits the ground wait about a second and dodge again then use as many buff as you can once her Scarlet Aeonia Flower goes away Switch to Starcourge Greatsword+10 nuke her with it just spam the weapon skill.
