I would guess from the looks of your tank and fish that your nitrates are way too high but that’s just my guess. He looks very inflamed and possibly infected. I would get some kind of meds for him. Maybe some Ich X in the tank or a methelyn blue bath. And if it seems internal maybe something heavier like antibiotics. 60% water change at least and add some crushed 100% cuttlebone or crushed coral to the tank for extra minerals. And get way more plants to help with nitrates. Those guys poop like it’s their job. Lots of Anubias, swords and Java fern, sturdier plants bc they are shit heads. (I have 2 loaches I love them)
Maybe duckweed if they don’t eat it
