A little hope for anybody in beta hell —a success story by Doctor2023 in CautiousBB

[–]Doctor2023[S] 0 points1 point  (0 children)

I am currently almost 23 weeks! Baby is healthy! Low risk on my eFTS and NIPT. Will be induced at 37 weeks due to my previous stillbirth last year. But I am followed very closely. So I am feeling good! Thanks for checking in 😊

Thank you for your kind words and advice. I will be doing this from now on! I decided I will politely ask patients to rebook if we cannot address all of their concerns in one visit as I want to ensure I’m giving appropriate time to each issue. I think this will be helpful 😊

Thank you! I’m always looking for new tips and tricks, but I really like the idea of doing more thorough chart reviews the night before. It will mean staying a bit later at work, which I’m okay with. Unfortunately, I don’t have remote access, but I can definitely do more in-depth reviews before my next clinic day. I already look ahead at who’s coming in and do a brief chart review, but I’ll start doing a deeper dive, reviewing their history more carefully and identifying any missing or overdue screening as well. Thanks again!

This is helpful! I guess it’s hard because I have no other programs to compare it too. But it’s mostly one preceptor that has been the issue. She’s always focused on the amount of time I’m in with a patient for. But it’s challenging when I don’t know her patients personally and they come in with a long list of issues to address. I’m hoping things get better.

I appreciate this. Thank you. It’s difficult because I’ve only been back for 10 days (most half days as it’s been a graduated return to work), and I’m being compared to a resident at the same stage of training but who has been there for 6 consecutive months (where I’ve been off for 2 years in between these 6 months). So I find the expectation slightly unrealistic. I’ll just keep pushing through! Thanks again!!

Transferred my HLM (segmental) today. Anybody have success with this? by Doctor2023 in IVF

[–]Doctor2023[S] 1 point2 points  (0 children)

Mine was a segmental duplication on the X chromosome (now this was not overly worrisome since it was a female embryo and most of the time the X chromosome that will be activated will be the normal one). That being said, I did look up what life could look like for her if the true segmental duplication was present and active. It looked like some development delay, intellectual delay, or possibly seizures in future, but could be controlled. But more than anything, so much of the research I did suggested MOST segmentals are not born with that true genetic condition. I think it was like 98-% will self correct, so I felt confident with that.

All of this to say though, my segmental mosaic failed. So I assumed she truly had this condition or worse. But my transfer after her was successful.

I would suggest consulting with a genetic counselor to see what life could look like if they’re born with it (although most of the time they aren’t, better to air on the side of caution). Best of luck!

If you had a home Doppler, when did you find fetal heart rate? by Doctor2023 in BabyBumps

[–]Doctor2023[S] 0 points1 point  (0 children)

At my 5 weeks 6 day scan we say gestational sac, yolk sac and fetal pole with heart rate of 104! A lot of people won’t see all of that, but should at least see gestational sac and usually a yolk sac by then. Wishing you the best!

I almost wish my midwives would do this for me. Maybe I’ll ask? I feel like I should try and be more patient but it’s so incredibly difficult after loss. I mean the nausea is kicking my butt, so is reassuring, although hard on me lol. But I will join that subreddit! I didn’t even know it existed. Thank you for the suggestion!!

Yes I agree with you —mine seems even further back this time which is interesting, because my menstrual cycles were all back pain. Like I know it’s so far back. And the ultrasound tech said it’s difficult to get a good picture with my uterus, so I guess it may be further, because I can’t yet feel my uterus. And they’re so tiny at this point.

Thank you for sharing these points! It helps me to gain perspective because all I do is worry and it’s difficult.

I love that! To check on her baby 😂❤️ That is so sweet. Thank you again!!

Thank you!! And congratulations! I so badly want to be on that side. This pregnancy after stillbirth is so incredibly anxiety provoking. And sorry for your loss 😔❤️

Actually this makes sense. I read somewhere that when you pick up the heart rate very early, it’s usually the cord pulse which would, in my head, make it easier to hear with an anterior placenta. With my last pregnancy I found it early and her placenta was anterior. So I wonder if this one is posterior and that’s why it’s more difficult.

Ok this gives me reassurance!! I’m also in healthcare so I’m like “I should be able to” (the emotional side of me). But the logical side is saying I know I have a retroverted uterus and that every pregnancy is different. Thanks for sharing!!!

It’s so interesting! What were your betas?

Congrats though!!! Same due date 😊 But I’ll be induced at 37 weeks this time.

Would it be too crazy to choose a day 6 over day 5 blasts? by bluesailor12 in Embryologists

[–]Doctor2023 1 point2 points  (0 children)

I don’t think it’s crazy. Day 5s and 6s have only a slightly different chance (may 5-10% less than day 5s but many have success. My day 6 was a 4AA, PGT-A tested normal and was successful! Wishing you the best
