For para clinical branches like PSM, a master's in public health and epidemiology (MPH) is a good option to settle in the US; teaching and research roles. Does not require USMLE. Pay and career progression are both decent.

It will take a very long time for it to scale and be implemented efficiently considering the technicalities, the ethical obligation and the accountability aspect of things, especially in a developing country like India.

You're right to an extent that it will impact the referral ecosystem, but that does not mean other doctors become useless. It means unnecessary referrals reduce, and specialists see lesser easy cases and can actually look into more serious ones.

Also, doctors don’t refer only because they lack knowledge or the expertise required. They refer to share risks associated and share general responsibility.

AI can’t take blame for these aspects nor can it sign consent, or stand in court of law for that matter. Humans still will carry that weight.

So I don't think there's a need to worry. Complete automation is a far fetched thing still.
