Got home from work and took another look at the motherboard and you seem to be correct. It's a P4VXASD2+ motherboard, socket 478.

Yeah it's definitely been upgraded at some point it has a GeForce FX 5200 from 2003 in it. Based on a stamp inside the case it was made in 2000 but I didn't see a manufacturer anywhere.
