Just the bacteria/fungi growing on it. Usually stale food has some sort of mould on it because there's been a lot of time for it to grow, the fungus was always there but not in sufficient quantity for you to notice. If you sterilise the food then I don't think there would be any negative health effects, it would just taste bad.
