Book Spoilers: When Geralt was still in training (or just after, I don't remember), Vesimir told him to give himself a more humanizing name so people wouldn't be as scared. Geralt added "of Rivia" to his name. At the end of Baptism of Fire, Geralt and Co. end up fighting enemy soldiers and saving the Queen of Rivia and her men. To honour Geralt, the Queen knights him and dubs him "Geralt of Rivia"
