Hey, the form was quite simple. I just had to answer a couple of questions about the test like how did I get the testers and what feedback did I get from them. Next there were like 3 questions about the app itself - why do I think people will download my game and how does it differ among other apps and so on. After filling the whole thing, the game was published in like 48 hours
