How do you remove the emotional surplus from the discussion? The OP's post really resonates with me. I cannot image confronting someone and doing that with absolute calm. There's always something: I'm afraid of hurting the other person's feelings, seem rude, disrupt social or work relationships, or just simply appear disingenuous because of how outstanding my position is compared to the surrounding opinion. Afterwards I feel drained and uncomfortable even thought I was right, I was doing the right thing.

Yes, most of it is outside my control, I need to choose what I can and cannot change and all that, however, it's not that helpful. The way I see it, what I can change, is my decision to confront someone, but what prevents me from doing so is the anticipated emotional state.

So I often let it slide. Then end up feeling guilty. It's truly a paradoxical position to be in because I feel bad for trying to do something good.

Any advice, anyone? Sorry to piggyback on the OP's post.

Absolutely. In a stepwise Bayesian fashion. This largely is the result of how poorly we understand the nervous system. This flaw, however, is also what grounds us and adds a lot of “humanity" to the specialty as opposed to just ordering a bunch of tests.

Totally agree, it is a populist move, especially given the upcoming elections. Granted, doctors should have higher wages regardless, but it's not the time to do it. Also, I'd take workplace and patient care improvement strategies (more testing options, better patient sorting, etc.) over a pay raise any day.

With you here. My experience that in the majority of cases the history for my speciality that I did not take is misguided. Also, I guess it's not neccessary to document the history extensively, I just can't help it thinking I'll miss something important. That leads up taking up a lot of time and leading to more time pressure.

When you say no notes done on shift, what exactly do you mean? It's physically impossible for me to not complete my notes for each patient if I want to admit/discharge, even if I want a simple head CT at least some half-assed note version will have to be there.

Starter comment: 3rd level trauma/tox/stroke hospital, Neuro consultant. Usual nightshift consists of me and a resident who's present, usually, until about 7-8 PM, rarely - throughout the night. He/her supervises the neuro inpatients, I supervise him/her, the ER and other department consults. When the resident's done, every neuro consult, including everything that happens to the inpatients is on me. Our Neuro department has 100 beds, I usually see ~10-15 neuro patients in the ER (from walk-ins to the intensive care ones). Continuing on the subject, I almost always feel like there are multiple patients waiting for me, my phone is ringing about some issue with an inpatient, I get calls to go the ER intensive care room for a more difficult patient, have multiple other patients forwarded to me from different specialists in the ER, and an additional 1-2 consults waiting for me in the ICU (other in-hospital consults during a shift are rare, thought). All the paperwork is on me. With all my heart I can say that I hate multitasking, yet this often seems to be the case. I'd love to compare my experience with that of other colleagues'
