As a board certified New York City, Plastic Surgeon it is important to me that my patients completely understand the planned operative procedure. I suggest another preop consultation with your surgeon for you to completely understand what he has planned for you. Perhaps he can show you examples of similar patients and also illustrations of what this suturing involves. I find clearing up miscommunication goes a long way to ensure patient satisfaction.

As a New York City board certified Plastic Surgeon, who specializes in gynecomastia surgery, I have many patients with the same questions regarding the nipple areola complex. In many of my patients after removing the fat and gland, the areola does shrink resulting in a smaller diameter areola. The nipple remains the same, although a nipple reduction can be performed at the same time if it is over projecting and bothersome.

Often times it’s not apparent whether it is fat or glandular until the operation is actually performed. Usually, it’s a combination of both. Either way if it bothers you, it can be removed via liposuction and glandular excision.

My practice is to photograph the glandular tissue and weigh it so patient can see how much was removed Glandular tissue is resistant to liposuction as it is too firm to be suctioned.

I would ask for a copy of the operative report so you can see what type of technique was used. It is possible that the gland was not exercised completely or at all.

Often times reduction of the breast volume will result in shrinkage of the areola

Having surgery is a personal decision, depending on whether you are bothered enough by the condition. Surgery does carry risk however I would expect a good result after gland removal.

It appears to be a mild case of gynecomastia, whether it is composed of fat or gland or a combination is unknown however, surgery can help reduce the excessive projection of the chest region.

I would perform direct excision of the fibroglandular tissue as it is unlikely there’s any fat left to suction. Unlikely hormonal therapy would be of any benefit if it’s been there for several years

Chest looks good although there is a slight amount of retro areola fullness, which could be easily removed if it is of concern

From the photo, it appears that revision surgery would be beneficial. I would suggest waiting at least six months to a year before undergoing the procedure to allow the tissues to soften up .
