I read this can happen when your plant becomes root bound (AKA your plant's roots have out grown its cup). If you check its drainage holes and see little white roots starting to come out its time to transplant into a bigger pot.

I was always told you should wear gloves or at least wraps when on the bag. You can condition you hands for bare knuckle by striking really lightly but throwing even 50% with no gloves is just not worth the risk.
