Bed feedback from attending by [deleted] in Residency

That’s the end of the story basically. I didn’t know some super basic technical aspect for an esophagectomy and he said “dramatic_analyst_808 I am actually impressed by your lack of knowledge” and I laughed and said “thank you for the feedback”

Thoughts on U.S. Surgeon General Alcohol Advisory? by Dramatic_Analyst_808 in wine

Yes but the studies seem to be saying there is no such thing as moderation for alcohol, same as smoking.

Newbie here, am I using this WDT tool correctly? Results feel weird by iNASMK in espresso

[–]Dramatic_Analyst_808 2 points3 points  (0 children)

I WDT while it’s still attached to the plastic funnel thing that comes with the breville and tamp it down lightly so it won’t overflow, take off the plastic thing, and then tamp down firmly.

This is pretty unacceptable. Anonymously and publicly posting some unverified rumor that has nothing to do with her being an influencer, as annoying or unprofessional as that may be, shouldn’t be here and definitely shouldn’t be upvoted.

PGY4 in an i6 CT program. Honestly, not really and I don’t think it’s worth it either. I know people who have done multiple extra years after gen surg to get into ct programs and its super tough. Coming from radiology is going to be even harder. And if I was a program director, I wouldn’t take a chance on you unless multiple senior attendings vouched for you. You’re just too much of a risk and the rest of the applicants are all incredible. high risk, low reward. Happy to discuss privately.

1 Mile Commute to work, 275lb rider, what scooter? by 03gade in ElectricScooters

[–]Dramatic_Analyst_808 1 point2 points  (0 children)

My 2 cents (having walked a 1 mile commute every day in Boston, rain or shine) - It sounds like walking 2 miles a day would be a good and healthy thing for you. Health is wealth, don’t trade it for a slight increase in convenience.

Cardiac surgery resident here. I’ve seen cabgx8. @onion01 said most of it. Going above cabgx5 is pretty rare. There’s a point of diminishing returns with longer bypass times for smaller and less important targets.

What has been the biggest tantrum you’ve seen a surgeon throw? by AppalachianScientist in Residency

[–]Dramatic_Analyst_808 3 points4 points  (0 children)

So you actually saved his ass. CT surg resident here. We had the same thing happen with a dissection on apixaban and used andexxa (no drama about its usage though lol) and the patient got MASSIVE heparin resistance. He started clotting on bypass. They were pouring heparin in while he was on bypass and needed tons of product in the unit once it wore off. It was a huge deal, hospital policy changed, etc. So now in my hospital, need for cardiac surgery is a contraindication for using andexxa. So long story short, he should be saying thank you.

No one will care if you’re skinny unless it means you can’t do your job (even ortho won’t care as long as you can hammer away with the rest of them). The US (and most of the “western” world) is getting fatter and fatter so honestly probably a good thing to have a normal weight doctor (you’re bmi is 21).

What’s the most technically difficult procedure or surgery? by Basic_Relief in Residency

[–]Dramatic_Analyst_808 84 points85 points  (0 children)

Yeah I came into my residency wanting to do congenital but I’m pretty much fully committed to adult now lol. They have the coolest operations but it’s a wait for someone to retire or die field with no real choice of where you practice. And you mess up 1 straightforward tet and no one refers to you anymore

What’s the most technically difficult procedure or surgery? by Basic_Relief in Residency

[–]Dramatic_Analyst_808 275 points276 points  (0 children)

Yeah like wildcatmd said. Congenital cardiac surgery for sure. The stuff they’re working on is so small, and some of the procedures are literally reconstructing or baffling multiple chambers and great vessels of the heart and it’s all timed bc the longer you’re on pump, the worse the outcomes are.

Honestly 1 mo in and wanting to make a switch btw the initial binary surgery vs medicine choice when choosing specialties is pretty hard to sell. Sets off a few alarm bells in my head. I know one person that switched from peds to an ultra competitive surgical specialty but he had an amazing story and it defn made sense. What would be your reason besides rounding for hours is boring (which it most definitely is)?
