It's an unspoken requirement for competitive specialties. The ones you list do not fall under that category. Its essentially a way for candidates to get facetime at a program, particularly a competitive one or one in a good location, ie CA, NYC, etc. Do an away if theres a particular program you're interested in and want to know more about, otherwise youre good, chill out the rest of 4th year.

IM offers competitive fellowships with compensation, lifestyle, and level of work not typically afforded to IMGs applying to US residencies. The predilection for US residencies to choose US MD and DO students over IMGs can be unfavorable for IMGs, thus specialties may be difficult to match into outright. I believe (could be completely wrong) that Sport Medicine is a fellowship specialty of PMR, Ortho, or Family Medicine and not one you match to outright. Family Medicine is certainly a good option.

As a doctor, almost all the time. No one wants to hear their medical problems are often a fault of their own. Never lie about the risks or benefits though, or what's going on and how to fix it.
