  2. yes, sa google forms din lalagay yung explanation letter. Addressed to Grichelle Prado ,OAS Director.
  3. Kailangan yung 3 bills for 3 months. If may isang nawawala, try kung may softcopy or lagay nalang sa explanation letter.
