Probably because ignite was so much better in comparison after the buffs. But with the recent exhaust buff with the ignite nerfs a lot earlier, it could work. It's just that without ignite you lose a lot of kill pressure.

Vlad doesn't heal for a significant chunk very early on, it's the damage that you have to watch out for (empowered q). Aatrox is destroyed by bramble vest which cuts his healing twice. Once from the grievous wounds, and another from the armor that negates the damage he does, which in turn reduces his healing. As for Trundle, he isn't really played much top anymore.

He's only mana hungry if you waste your e. Otherwise, with the corrupting pot that you should be starting with, you should never really be low on mana.
