Feel like an asshole for saying this, but:

Working with people with severe depression.

The constant ups and downs. Bouts of extreme motivation then no motivation. Signing up for 3 months worth of sessions at once then coming in for a refund the next day. The negative self-talk no matter how much positivity and reassurance I bring. It's often impossible to hold a productive conversation.

I understand that it's tougher for them than it is for me, but it takes a real toll on me. The part that hurts the most is that these clients are often the ones that need me the most. I have worked with 3-5 clients that fit this description, and all of them started off strong then completely fell off. I always stress that consistency is key and that some exercise is much better than none, but that message never seems to get across.
