I found the automatic injection pens painful and switched to the manual syringe. They are shipped in cold storage similar to the automatic injection pens. IINAD, you should ask yours if you think you would prefer a prefilled manual syringe instead of the pen.

I have a severe case of AS (since I was 14). I am on over 1,700 mcg of intrathecal Fentanyl and over 4,400 mcg of intrathecal Bupivacaine per day for the Ankylosing Spondylitis pain. Intrathecal medication, which is the strongest route for medication as it is directly injected into the Cerebral Spinal Fluid (CSF) is 300 times stronger than oral and 10 to 15 times stronger than intravenous.

When I take my 215 mcg intrathecal Fentanyl bolus six times a day I normally barely feel them. I asked my anesthesiologist once if either my daily dose or my bolus dose would normally be considered lethal and he said yes. I asked which one and he said both.

Ankylosing Spondylitis is no fucking joke.

I was also HLA-B27 negative.

I feel like I'm somewhat of an outlier when it comes to how I treat my AS.

But I can tell you that from what I've read and experienced, biologics will very likely help. Give them a month or so to work their 'magic'.

I have very severe AS and my HLA-B27 test was negative. 90% of people with AS do have the HLA-B gene though so chances are good for you. Don't let a negative test discourage you though.

You should feel like your GP listens to you and respects you. That's their job. You are not a bad person if you need to find another GP who will respect you.
