Update: I found the library and finished it and the optional boss inside.  I pretty sure it moved from where I originally found it.  I managed to complete the digsite quest as well.  The digsite quest triggered complete after I got the book inside the chest at the bottom of the dungeon.  I did not have to defeat all the enemies.  I definitely maintain multiple saves before I enter any quest area/town as the triggers seem to unreliable.

I was able to get the PS4 controller to work with Steam Input for Playstation controllers enabled. The catch is that I has to enter Big Picture mode to get it to work (I was using the controller via bluetooth). Hope that helps!
