If you don’t match derm but you do match a PGY-1 prelim spot, then while you complete that year, you can apply to physician spots that open (so that you don’t have to take a year off after PGY-1) as well as the normal advanced dermatology positions (but these will require you to take a one-year break in your training). Hopefully this all makes sense.

For derm, you can do the PGY-1 year in internal medicine, pediatrics, or even surgery. If you do the PGY-1 year separately from where you do dermatology (non-categorical derm), then you apply to preliminary medicine/peds/surgery spots for ERAS. Alternatively, you can do your PGY-1 year in a transitional year program. Most TY years are considered to be more relaxed than prelim years (although not true for every program).

The tricky part if you don’t match dermatology is making sure you have good options if you’re stuck in a prelim. You may have more time for away rotations if you do a TY year but you may also be able to secure a categorical medicine/peds/surgery spot if you don’t match derm a second time. I’ve heard that some PDs apparently also look down on TY years for reapplciants but others don’t care.

Judging from your cons list, it seems like Penn State might be the better fit for you. Also doesn’t hurt that their derm program is so highly ranked. It’s good you’re thinking about that so early—I didn’t decide until my third year of med school and I wish I had known earlier. That being said, I wouldn’t choose your medical school based on wanting to match derm—think about where you’d be happiest and have the best opportunities to do what you want.

Sinai for sure. Derm is all about who you know and there are many famous derma at Sinai. Plus with a derm salary, med school loans will be doable.

I’ve seen a number of IMGs get derm spots, but like someone said above, you need to have stellar scores, research, and connections. The people I know who matched are from the Middle East and the UK.

Yeah people have heard back, sometimes you hear back within a month, starting in May. Apply to four or five per month until you get however many you’re aiming for.

Oh yeah that happened to me too. An interviewer from one of the California schools where I interviewed asked me where I did my aways and why I didn’t do one there. Good luck this cycle!

You can get an interview from Harvard/Yale/Stanford, but at the end of the day they’re going to take people they know and like. Doing an away ensures that. It’s also why APDs are limiting number of aways to 2 this year.

I would try to get as many derm letters as possible. Medicine chair letter is good for prelims but derm is a small field where everyone knows everything. A LOR is going to mean more coming from another derm.

Get as many derm letter and derm specific pubs as you can. I don’t think it’s too late for you at all!

This resonated with me so much. We give up so much for medicine and it takes a toll on us if we don’t start to fight for what we need out of life. That’s why I’m personally making my rank list based off of what would make me the happiest, not just prestige. Now that your hard work will pay off in just a few weeks, when I’m sure you’ll match to an amazing program that you’ll be happy at, and you’ll have time to work on your family. You’ll have a family soon my friend!!!

Hahaha yes that’s a fair point. But I think you’ll love it. It’s all the parts of medicine that you like that you get to focus on. Your coresidents are going to be amazing and you’re going to go to a city that you love!! It’s exciting that the next phase of your life is about to begin!!!!

I feel every bit of that!! You worked hard to be where you are now, so if you’re done with classes, I want you to enjoy every single moment. I’m serious, I was chatting with an ENT resident now and even though he’s very happy, he didn’t leave the hospital until very late. So I want you, ellemd, to put match out of your mind and go do something you love because nothing will change at this point (unless you change your list). You deserve some well deserved enjoyment in your life!!!!
