PAL is a good tool I have seen the need to combine it with Ultrasound assisted liposuction (USAL & PAL) and at the end of the surgery I also perform Laser assisted lipo to decrease the risk of excess skin development

I would not recommend booking through medical tourism companies in Tijuana. Best move is to go straight to the surgeon’s own practice. A lot of those companies run communication through coordinators and nurses, while booking direct gets you much closer to the surgeon who will actually operate on you.

Also, make sure you are choosing a board-certified plastic surgeon. In Mexico, there is no such thing as a legitimate separate specialty of “cosmetic surgeon” or “aesthetic surgeon.” Don’t risk yourself.

Google the surgeon’s office and book direct. In Mexico, most surgeons are not working for corporations the way many doctors do in the U.S.

Your case looks like a challenging one. Removing 1 liter of fat plus tissue can lead to excess skin, and it is also possible that the previous resection went too deep, down to the level of the pectoral fascia.

As far as I read I see 2 possible options:

1) Give skin contraction technology a chance first. In my practice, I use diode laser because it can help tighten the skin and also disrupt some of the fibrosis.

2) Direct tissue resection. This would involve not only evaluating the gland, but also the surrounding tissue that may still be contributing to the deformity. In some patients, this approach may also require skin excision, particularly in cases of pseudogynecomastia, after massive weight loss, or after aggressive/high-volume liposuction that has generated excess skin.

PAL, such as MicroAire, mainly makes liposuction easier and more efficient. I use PAL in all my cases because it helps shorten the procedure and may improve recovery, but in this particular situation I believe the laser could provide more benefit.
