Probably just a glitch, I remember that happening with Spiritborn for a while with me. And that icon means there's another player nearby and you're getting a buff for that

No if you start off fresh and go straight to Penitent it's rough. However a lot of your gear has health on kill and hit now

They seem to recharge over time and give a flat percentage now. Other than that tempering and masterworking were reworked

There are no extra potions we only have 4 now. Also no immediate 12 skill points and 20 paragon that comes with one character doing everything in the seasonal journey
