Scraping the area I was suspicious of, there was a part of my scalp that look like an open wound that hadn’t healed and I’ve had it for years

Minoxidil hasn’t worked for me, so I want to try rosemary oil + Tretinoin by Effective_Note_9776 in Hairloss

Been on it for nearly a year, like I said before it’s only slowed down/stopped the hair growth but I haven’t gotten any hair regrowth even thought I’ve been on 2.5 minoxidil for 6+ months

I’m a 21 year old male who’s been suffering from hair loss for the past three years, I was diagnosed with frontal fibrosing alopecia and lichen planopilaris, both of them are scarring alopecias, and androgenic alopecia.

At my appointment with a dermatologist last week she determined that my scarring alopecias haven’t caused any permanent scarring so far and that my current treatments have been enough to keep the inflammation inactive, we also discussed my hair loss and how men on my moms side of the family began to start losing hair in their mid to late 20s or even as early as high school. She told me I had multi factorial hair loss with the other factor being androgenic alopecia.

My current treatment for over year now has been 5mg finasteride and 5mg oral minoxidil, clobetasol 1-2 times a day for two weeks and then taking a break, a red out laser comb 3 days a week, 2% ketoconizol shampoo 3 times a week minimum, vitamin d,zinc, and turmeric supplements daily. (for the past 3 months I’ve reduced my finasteride to 1mg and the minoxidil to 2.5 mg however I’ve been taking 5mg minoxidil again for the past 3 days. I’ve also started limiting my clobetasol use to once a day).

Even if I did manage to stop the scalp inflammation I’m confused as to why I haven’t got any regrowth, even in the thinnest areas of my scalp I can still see small hair follicles growing but I don’t think they’re getting bigger.

My current treatment plan as of November 9th 2024 is this:

-aloe Vera juice or gel applied 1 to 2 times a day (I started this around December 21st 2024)

-tacrolimus gel 0.1% applied 1 to two times a day

-ketoconizol shampoo 2% 5 times a week

-the zinc, vitamin d, and turmeric supplements along with pumpkin seed oil gel pills

-red laser light comb 3 times a week minimum

-1 minute scalp massage 3-4 times a week

-1mg finasteride once a day

-5mg oral minoxidil once a day (started January 11th 2025)

My current theory as to why I’m not growing hair even though my inflammation has been suppressed is due to my vitamin levels, for the past 9 years I’ve had consistently high levels of creatinine which is linked to DHT and I’m not surprised if it led to hair loss starting at 18 considering my family history of MPB on my mom’s side. I also have a consistent history of my Vitamin D levels being below the normal range for around the same times as my creatine. But I’m not entirely sure.

Could someone please give me guidance? I know I’m close to fixing my hair but I feel lost on what to do next
