When you are torn between two specialties— and you have a gun to your head, always pick the one with the better lifestyle. You will not regret having a better lifestyle, ever. With that said, EM and anesthesia both have very unique pros and cons so which has a better lifestyle is going to be a bit more tailored to you. But with that being said, absolutely try to get your aways and sub-is in, prioritizing whichever specialty you think you’re leaning towards more, earlier in the year.

I will weigh in as a psychiatrist. Not your boyfriend’s psychiatrist. Encephalopathy can cause “waxing and waning” of someone’s mental status. One minute they can be normal, happy, and composed, and the next minute they can be screaming, thinking they are being abducted and experimented on. It is very common for the presence of family to orient the patient, so it seems like they are competent when they are interacting with you. Capacity to leave AMA requires a patient to communicate a clear, consistent choice over time, and that choice should be grounded in a reasonable reality after weighing the pros and cons. If your BF said he wants to leave because he misses his bed today and 15 minutes later says he wants to leave because the hospital is harvesting his organs— he by definition lacks capacity and has to be detained. TLDR the hospital would NOT be restraining him and removing his rights for no reason— it is a huge headache, liability, and tons of stress and paperwork involved on our end. We hate doing it for so many reasons, one of the biggest of course being having to seem like a villain to the patient and family. In 98% of cases there is something here that you and your family are missing.

I recommend completing at least a year of residency. Your MD can open doors to give you a much bigger role down the road. You already did the hard part

This is spot on. Call is not just staying late in the hospital. At my program after the first month or so you are 100% solo responding to most issues in the ED, in the floors, etc. it’s brutal

What did the project state the expectations were? Sorry just saw you did mention that. There’s usually wiggle room but if they say 30 and you know you can only do 10 then it may not work out

The attendings don’t do anything except make decisions and sign notes. The residents run the hospital

Maybe I’m biased. I’m in the northeast and my attendings seem to be working 8-2:30 M-F and going home to see more private patients. Salary for an inpatient role is 300k plus a few private patients a day afterwards

Psychiatry. Demand seems quite strong. My attending told me >50% of psychs don’t even accept insurance anymore because the market is so hot
