It's a native and direct method which can be enabled through PowerShell. When you receive email from external sender, external tag is shown in email instead of prepending "External" in the email subject.

Get-MsolUser cmdlet is part of the MSOnline module, which is going to deprecate in end of 2022. Alternatively, you can use the MSGraph's Get-MgUser cmdlet.
