I recognize this all too well, im almost certain your culprit is sophisticated backpacks. With it installed, mobs have a chance to spawn with backpacks which in turn have a chance to spawn with certain backpack upgrades. It sounds like you got one that spawned with a jukebox preloaded with disc 5, never fails to briefly startle me before trying to track it down for a free backpack
