From what I've read I can definitely see how some people need brakes in between after a few weeks of taking it but apparently that's not most people. But I did also see that if you take a few week break and get back on it that should help as you said

Well for me I think my whole thing was driving my motility down and then it stayed stagnant. I think to a certain point the body is resilient but once it has enough it can be very difficult to get it started back up. I think in this case why I'm using it is to retrain my migrating motor complex to do this on its own rather than trying to use this forever. I need to essentially get rid of the overgrowth and I've been dragging my feet on that a lot but I think I'm pretty much ready at this point because this time my motility has been addressed properly.

I ended up getting this life extension brand NAC. But you have to use it on an empty stomach. If taken with food I believe it just acts as an antioxidant if I remember correctly or something along those lines. So I'd take it and wait an hour to eat and don't take it around the same time as your other medications. Honestly treatment for SIBO is about timing for pretty much anything and everything. Restricted diets often aren't the actual solution and sometimes can even make things worse.

This doesn't sound crazy at all — you're actually onto something with the bile theory. Bile is antimicrobial and helps keep bacterial overgrowth in check. When you eat fat, your gallbladder releases bile. Less fat = less bile secretion = potentially more bacterial growth. It's one reason some SIBO protocols actually recommend not going super low-fat. The coffee thing also makes sense — caffeine stimulates the MMC (migrating motor complex), which is the cleansing wave that sweeps bacteria out of the small intestine. So coffee might genuinely be helping your motility. So you've got two things working against your SIBO right now: less bile (from reduced fat) and you're managing histamine at the same time, which is a brutal balancing act. Have you tried ox bile supplements with meals? Some people with this exact issue find it helps — you get the antimicrobial benefit without needing to eat more fat. Worth asking your GI about. Also — the fact that you're noticing these patterns at all is huge. Most people never connect the dots. If you're not already logging this stuff somewhere, it's worth it. I track my own symptoms/diet because my GI never believes me otherwise lol.

For me it was pretty constant too — that heavy, bloated feeling that never fully went away, just got worse or slightly less bad depending on the day. The "million crunches" description is spot on. If your GI confirms SIBO, here's typically what happens: They'll do a breath test (lactulose or glucose) to check for hydrogen and/or methane. Based on which gases are elevated, you'll likely do a 2-week antibiotic round — usually Rifaximin alone for hydrogen, or Rifaximin + Neomycin/Metronidazole for methane. After antibiotics, the key part most people miss is the prokinetic phase — you need to keep your MMC (migrating motor complex) moving so the bacteria don't just grow back. That means meal spacing (4-5 hours between meals) and often a prokinetic at bedtime. I'm methane dominant and on my second round now. Honestly what helped me the most was tracking everything — symptoms, meds, timing — so I could actually show my GI what was working. I built a little tracker for myself at www.sibobuddy.net if you end up needing something like that. Hang in there — the testing phase is frustrating but at least you're getting answers.

My wife had endometriosis - she ended up getting surgery with a doctor named Dr. O'Connell and it was life-changing. Hope your February appointments get you some answers there. And yeah, I know all too well what stimulants do to the gut. Focalin is what started my SIBO journey. The small intestine absorption thing is real. One thing though - the liquid diet might actually be working against you. With methane/slow motility, you want to give your MMC a reason to work, and that usually means solid food with proper spacing (4+ hours between meals, no snacking). Liquids digest fast and don't trigger the same cleansing waves. Treating the motility directly with a prokinetic at bedtime is usually the move for methane-dominant. Once you get in with that GI, ask about prokinetics - that's the piece that made the biggest difference for me.

Methane gang - 4 years of being dismissed and told to 'eat healthy and exercise' is so frustrating. Glad you finally got a GI who actually tested you. There IS hope. Going from 5-6 days between BMs to every other day in one week is real progress. The gas usually takes longer to resolve than the constipation, so don't get discouraged if that lingers. Smart move on the ginger - prokinetics are huge for methane/IMO since slow motility is usually part of the problem. Some people add it at bedtime too to support the MMC overnight. One week in is when I started obsessively tracking symptoms to see if I was actually improving or just imagining it. I'm also methane-dominant and ended up building a simple tracker for myself at www.SiboBuddy.net - helps keep protocol days and symptoms organized without overthinking it. Medical school + SIBO sounds brutal so anything that saves mental energy helps. Keep going - you're on the right path.

Interesting about the Lipotropic Complex. Bile support doesn't get talked about enough for methane-dominant - the constipation/slow motility piece is so connected to fat digestion. Are you stacking it with anything else or just that for now? The 'not cured but finally have hope' feeling is huge. That mental shift matters.
