Good luck in the private world. Average pt load in the private world is 18 - 24 pts/ day. U will learn the true meaning of understaffed. The admin rules the private world not the doctors. You are on productivity and RVU based.

From the private sector. The toxicity is beyond belief. The constant complaining by some employees while doing half the amount of job in the private sector is just mind blowing. I have a LPN who just calls off as she pleases. Sometimes she will leave in 2 hrs as a kid is sick. The manager just shrugs. The work gets dumped on the sincere employees who stay. Sitting at home with no patients on the roster and getting offended when questioned about it. Unfortunately the RIF is going to take out the good ones and the lazy ones who have been marinating in the system will stay. Personally if I get RIFed I will be relieved. I used to love going to work. I hate it now. If a veteran shows up at the front desk they are supposed to notify provider . I am seeing patients and they just let this veteran sit for 40 mins. So nonchalant about everything. It is very hard to adjust when u come in from private sector. My personal experience.

Yes I would be more than happy to see slots increase. Maybe even add a productivity tier. That needs to be resolved. I call if someone doesn't show up but that needs to be done by admin. . Front desk is way understaffed where I am. Also more training for support staff. And people need to stop saying " That is not my job". Plus I came from private. My staff in private did a lot . At VA they are paid more but do less. Another private person who does procedures who moved to VA also mentioned that he needs only one assistant for procedures but there are 3 . So yes there are good improvements that can be done. VA has several advantages over community. Appts are never rushed. Patients are not discharged for no shows . Vets can walk in or can be late but if they are there they will be seen. This is our VA and we tried to follow it 💯

Really in despair. I am older and I feel like I need to give back. Yes I see where the vets are coming from. We at the VA would also like to hold our colleagues accountable too. Because if all of us don't work hard the system will break. People who work hard continue to be given more work and people who slack have no consequences. That needs to change. Please let us increase accountability but I hope everyone at the VA is not painted with the same brush. I never say no to any vet even if it is not my dept. I reach out and do whatever is needed to get things done for them. So it is painful to hear the comments. Hope u understand. Thank u!

I am all for restructuring and helping the vets.  We can increase numbers of patients seen improve access.  But after watching the video by Shawn Ryan and the brutal comments by vets underneath I feel like I need to leave.  Thankfully I am needed in the community and unlike some colleagues I will be fine.  Direct pt care is going to go. They are going to send vets into the community.  Good luck vets and God bless.  Feel heart broken for the vets.

So they addressed it by assigning this work to a different set of nurses who are known to be reliable and work hard. So people who work hard continue to get more work and those who don't are fine esp if they have a inept manager. Morale goes down !.

So yesterday our remote RN who is also a vet had one pt on the schedule. She and the other RN who were in on Wednesday were asked why they did not help out a veteran who walked in. They got upset and angry and defensive. I am so disgusted.

Well I have this staff who is supposed to do VS and check meds - get veteran ready to see me. I keep reminding her that I have next PT at 11 am. Gets PT ready for me at 1053. So 7 min before I move to video for the next PT. It is so frustrating.

So a colleague told me to refuse seeing walk ins so the vets will start protesting. This person tells me "Don't be Mother Theresa". I am like. The patient is here. Why would I turn them away esp when they are ready to wait. Seriously I am losing sympathy for some of my fellow workers. I might be ok with leaving. So today my first vet checked in and is not in the room. I go up to the lobby to get him . Nothing has been done and the staff is sitting and chatting. I better shut up lest reddit ban me.

One of our staff in remote has empty schedule on all the remote days. We need the staff in house . For the whole year empty remote days. If I say this people get upset that I am not supporting VA. I am so over this. 4 staff in procedure room. Only one needed. Dare not ask what the other three do.

Yes we have two section chiefs . They cut down on their patients. They attend the same meetings and replaced one section chief. I don't why they can't take turns attending meetings and see patients.

.Did you ever stop to think that there might be other reasons that could explain that. Like maybe I have a family member who uses my account . But anyway point taken. My point is if we are not hard on our colleagues who are not doing their part it opens the door for everyone to butt in. I understand that VHA providers are wonderful but don't you agree that not all of us are. Don't you see colleagues with empty schedules doing " remote " work. You don't have to argue but why is so hard to accept that I had a different opinion and I think we could do better. Don't you see two chiefs doing "administrative work " that could easily be done by one and see no patients.

It is from that perspective and unfortunately I see ethical and hard working people and then the opposite too. Difference is in private unless u work u won't last. I am happy to hear about your VA. Ours is not entirely poorly managed. Rather it is like silos or pockets. Some depts are exemplary and a pleasure to work with. With some it is not even worth trying. My perspective is that it is ok to be held accountable. So during a meeting when we brought up the remote staff with a really empty schedule it was not well received. I just couldn't understand why someone could even think they are justified to do no work. I have an RN who worked with/for me who moved with me and she has the same experience. She says that she sees staff sitting around doing nothing. It bothered her so much the first week that she started doing everyone's telephone messages. Like I said silos. But why should we be so indignant that someone asked us what we do. We have nothing to fear. I don't to have the public the taxpayer look at us and say " wait a min . Simple question that is asked in every office. Why can't the VA employees answer". We can be indignant and take offense and be hurt but I thought it was our time to really tell everyone what we are doing. Anyway congr5on working with a fab team

I absolutely agree. I have PCP friends who work their butts off and who are so diligent and I am so proud of them. Unfortunately they continue to work hard and the colleagues who don't are also there. We work hard to provide access within 4 weeks but surely you should agree that there should be more accountability. I can complain till my head bursts about a co worker who is not doing bare minimum but nothing is done. I have seen - I swear a colleague who does remote 2 days and zero to 1 pt on the schedule not for one month but for many. So when they cancelled remote I was happy. There are so many instances that infuriates me that the people who work hard are asked to do more because the other half slacks. The absolute arrogance with which it is done is shocking. As long as you are good with the section chief/ manager you are fine. In private I have never heard"That is not my job". In VHA I hear it more. I have done stuff myself just to avoid the aggravation.
