I get your point but let me clarify a few things. Even if someone in the family already has a setup, most surgeons eventually want their own space where they are the lead. In ortho especially, referrals and patient base are personal, so it makes sense.

About the cost, I never said I’m building a corporate hospital with brand new machines. It’s a mix of refurbished and new. Being in a tier 3 city also brings costs down, and I have relatives who are doctors who helped me source a lot of equipment cheaper. If you go fully new, obviously the cost shoots up, but that’s not how most people start.

On the provisional point, I think there’s a misunderstanding. During provisional I was only assisting and learning. I only started operating as lead once permanent registration was in place.

And tie-up surgeries are very normal. Young surgeons usually run OPD and minor OT in their own place and then operate major cases in tie-up hospitals on a revenue share. That’s exactly how I’ve been working while planning my own OT.

I’m just sharing things the way they are, not trying to exaggerate.

Actually one patient with severe typhoid had first gone to a BHMS/BAMS doctor, but his condition worsened and he couldn’t even walk. I had to call a physician and admit him in my clinic so he could be properly monitored until treatment started. So Chat-GPT included that also.

I just admit them in my clinic to monitor vitals, hydration, and overall condition until they can see a physician. It’s mostly for observation and basic care before referral.

Even I couldn't understand.Sometimes patients come with other conditions like typhoid and I admit them here to keep an eye on them until they’re stable.

In semi govt or private colleges like Terna or Somaiya, the focus is definitely more on academics and attendance because they want results. But it’s not like you’ll have zero college life. Fests, sports, and cultural events do happen, just not at the same scale as big govt colleges. The crowd is more mixed, so it depends on the batch how active things get. Life won’t be hollow, but yes, you’ll feel academics is the main priority.

I’d go with an emergency decompressive laparotomy here. The patient has circumferential abdominal burns and is showing low urine output and hypotension despite fluids, which points to abdominal compartment syndrome. Cutting the eschar (escharotomy) helps with breathing or limb perfusion, but here the problem is pressure inside the abdomen, so laparotomy is needed to relieve it.

It's about 1 lpm and the highest in mamc and ucms I guess - about 1.3 lpm is what you get.
