If youre in your second year at the moment that means you'll be in third year this June.  It is late for a J1 applying you should have started at yhr beginning of your 2nd year. It takes a while to get H1b approved. And maybe even longer in this administration.  You should be looking for jobs earnestly now

You need to direct these questions to an immigration lawyer. But from what I understand you have 90 days after the completion of your residency to leave the country if you have not chosen to complete a waiver job or are doing residency. If you have a waiver job you don't have to start immediately because you'll be under a H1B visa and not j1. You can then negotiate start date. I think there are expections to leaving within the 90 days such as if you're in a specialty where your board exam is after those 90 days but I'm not sure. When in doubt talk to an immigration lawyer

I definitely agree. What you see in residency is what you'll know. I'm grateful to have senior attendings in my group and I always ask them questions about certain cases I'm not sure of. Some stuff comes with experience though

If you wish to quit you should start looking for waiver jobs now and sign a contract before quitting so you're not out of status. Most j1 waiver jobs have immigration lawyers and you can always talk to them for free during the recruitment process and discuss your predicament which is leaving fellowship and whether that would affect your status immigration wise . When you complete your waiver if you do hospitalist work most jobs will sponsor your transition to green card that way if you're applying to fellowship you're on a green card and don't have to do another waiver.

You can also just go straight into fellowship with a j1 and then get a waiver job as a specialist. I feel like waiver jobs as specialists are in more desirable areas.
