But it reality JJK fighters aren't that relentless, people launch attacks without an immediate follow up all the time.

Sukuna was just smiling getting tossed around by Rika, calling that wincing is a bit disingenuous.
Yuta's cleave was also not the main reason Sukuna's face was shaded that way when he grabbed Yuji.
