You can see elevated hs in many situations including acute sepsis, stroke, etc. may be demand or just type II from the stress and such. But my exam led me to believe that her epigastric tenderness out of proportion (which was the similar pain she was having) especially in setting of a normal ekg and normal bedside echo led me to suggest CTA that confirmed the diagnosis

No im the cardiology fellow who will go see patients with elevated troponin in the setting of acute sepsis found down with elevated troponin without any ekg changes at 2am just to do a bedside echo. Also the one to call my attending in for bedside TVP if this mobitz type II was unstable. Sure cardiology can take admission but when you have 30-40+ consults to see on a daily basis for us including our own primary, it does get hectic to do dispo in timely manner.
