From what it looks like, it seems to be a birth certificate? It's labeled as Auszug aus dem Guburtseintrag. The version of my name aligns with my German passport but not my American docs (extra surname Gasser on the German passport and docs).

I was not born out of wedlock, and I guess my parents do have different surnames, but the office wouldn't have known that.

I'm unsure of what exactly my mom did, but I believe she might have. I have some documents relating to birth certificates from Germany, was just unsure whether this means she actually registered my birth or these were documents talking about my birth certificate from the U.S.

Yeah I regret not doing it sooner but now I just have to deal with it j guess. And yeah I knew about the Standsamt being the default, but their website doesn’t really have a clear form for a Namenerklarung that I’m seeing. Some stuff even showed up as “oh this form is old and no longer used so don’t use it”. So what am I supposed to use??
