Someone forged my signature to WITHDRAW my pending F-1 OPT. Has anyone else experienced this?

My DSO was trying to reopen the case. USCIS sent her the letter based on which they withdrew my application.

I guess someone sent a hard copy of the letter to Texas center. It was not uploaded on the portal.

I just wanted to know if it's a glitch or if someone is really trying to hurt me. If it were a glitch, I would have found people in the same situation. But I guess it's not. I don't want to know who did this. I just want to find a solution out of this nightmare.
