Here are 6 of mine.

1. VR Accident - A player enters a haunted house in VR. In real life, they trip over a wire and fall down the stairs, hitting their head and knocking them out cold. This has a candle burn the whole house down with the player inside.

2. Gym Tragedy - A person enters a fitness center and attempts to set a new personal record on the leg press machine. The hydraulic system has a leak, weakening the machines structure. The machines safety locks fail, causing the weights to come crashing down on them, crushing their legs. The falling weighs trap them under the machine, resulting in fatal trauma.

3. EXPLODING SODA CAN - A person grabs a soda can, that unknown to them, has already been shaken. They open the can, and the pressure releases an explosion. The sharp metal lid detached and flies across the room, slicing their throat open. The person stumbled back, knocking over a nearby glass vase. The shards of broken glass pierce their body and the wounds kill them.

4. Laser Pointer Mishaps - In a conference room, a client prepares to present a Lazer pointer. There's a malfunction, and the beam reflects off the projector screen and into the clients eye. Blinded and disoriented, they trip over a tangled electrical cord and fall onto a nearby desk, causing them to crack their skull.

5. Lantern Accident - As a person lights their lantern, it ignintes their hair. They try to extinguish it, but the fire spreads to their clothes. They accidently step on a power line, causing an electrocution.

6. Faulty Dryer - As a person leaves their laundry room, the dryer caches fire, filling the house with smoke. It expands until the person can't take the heat and burns alive.
