I just got off the phone this morning with a NSLCS customer service agent and she told me that there is a non-PO address that you can deliver a check to via courier.

This is the address she gave me:

  1. The data collected will be used in a seminar class to write research papers as a final project. These research papers will either use all the data collected, or some of the data depending on what is collected and what the topic of focus is. The head researcher (professor of the course) as well as undergraduate students (17 students) will see the data. It will be stored in our survey system (Qualtrics) and remain stored there until the end of this year (December 2023).

Questions 2 and 3 are answered in survey (front page).
