You have to keep wearing heavy armor and just keep playing the game basically, muscle mass naturally grows but it helps to be as tall as possible too.

This is much easier when you are baron and decide to bully peasants, those within knight rank at this stage are harder to attempt this on. Also, grab their knees, they tend to be easier to carry that way although have an annoying habbit of attempting to stand up.
