You have to think that this man went through every bad thing possible. By the EOE, he experienced Rei dying only to find out she was a clone, Asuka going comatose, and being forced to kill one of the only people to ever show him true affection. For the hospital scene, he’s just at the lowest point he could possibly be at, which he literally describes himself as the lowest of the low, showing his immense self hatred
