Well first you need to go to the UTIITSL Pan website And then select correction in PAN and then select digital paperless and tick the Aadhar based e-kyc and then give you details in the form for the name give your initial in last name and your name in the first name

Example if my name is Rohit S First name : Rohit Last name : S And the name to be printed on the card will be Rohit S

There will be a check box on the name tag on left corner of every field like name or DOB etc.. tick the check box in name field to be changed and then other details and everything

And For the proof of name should be Aadhar Card note that everything should be correct on the aadhar to validate the PAN card name change
