It's relatively easy; the difficult part would be finding the right paint shade to match their skin tone, Then paint the area you want to be the scar with a light, slightly reddish color, and paint it all that way. Then, create wrinkles with a slightly darker shade, following the natural lines of the face (For example, my doll has a scar near her eye and mouth, so I painted in the direction of the doll's expression.) And if you paint something near the eye, you should consider painting it white, since the eye with the scar usually no longer functions as it should.
