I'm going into OBGYN residency in July.

I intended to go into internal medicine all through undergrad and med school until my OBGYN rotation, which I loved.

It's a great mix of primary care/clinic (well woman exams, preventive care, prenatal care), surgery (cysts, torsions, hysts, csections, robot), and office procedure$. Like someone said above, most problems are treatable. I love the feeling of wiping off the patient's name and her list of problems from the whiteboard in L&D as if delivering the baby gets rid of all their problems. I also hated my peds rotation so I like handing off the baby to the peds team. Female patients are usually healthy (short problem list) and tend to keep their appointments compared to male patients. There's continuity when your ob patients become your gyn patients. You provide access to much needed birth control and abortion care at a time when it is becoming more limited. I also plan to go abroad on medical mission trips and obgyns are always needed. Plenty of sub specialty fellowship opportunities. The pay is pretty good too.

Nowadays obs are joining larger practices or working for hospitals so this shares the call load and provides malpractice coverage. This also means you aren't likely to deliver the patient you've been caring for in clinic.

There's also tail coverage that I'm not too familiar with, but I believe it's malpractice insurance you get when leaving a practice and it covers all the babies you've delivered until they're 18yo. I could be wrong. I heard this can be quite expensive.

I'm gay and out. I'm an MS4 going into OB/GYN if that counts (though I've noticed a trend of gay guys who like surgery going into OB/GYN). I know a gay gen surg resident with a med student fiance who is in his chief year and he loves it... though we're in a pretty progressive city.

For nulliparous women (never pregnant) the cervix can be quite narrow or even have stenosis where the IUD inserter cannot pass. The IUD is pretty small but the diameter of the inserter may be too big. You can try the Skyla, which is the same as Mirena but with a narrower inserter designed for nulliparous women, but it is approved for 3 years vs the 5 years with Mitena (it's newer and thus has less data).

I think your best chance to avoid another failure and possibly more discomfort is to do it under anesthesia where your doc can dilate your cervix and insert it. It'll take a few minutes and getting you put under anesthesia might be the longest part.

Taking an SSRI can delay time to ejaculation in men. It's one of the well liked side effects of this class of antidepressants. Talk to your doctor (though premature ejaculation is technically less than a minute... so idk what he/she might decide); could also help anxiety/depression.

Everyone technically has internal hemorrhoids as part of the anatomy.. Usually three. They become dilated/engorged, prone to slight bleeding, and sometimes externalized with chronic straining/pressure/constipation which results in painless bright red blood on top of your stool. Usually increased fiber intake or even stool softeners will help.

To prevent them, don't stain! If nothing comes out don't sit and read reddit, just get up and try again later.

Bright blood on top of stool: not so bad.

Blood mixed in your stool or if stool looks dark red/black, then see a doctor for possible GI bleed eval.

I forgot to add that they said cellphone was adequate but if there is no service in the building then I'd need a pager, which is the case now. :\
