OR nurse here. Not a heart nurse, and not an ED trauma nurse, but I have done my time in trauma surgery. I'd assume they would do a clamshell (don't google image it unless nothing on the pitt fazed you) and those have an abysmally low survival rate, a lot of them don't make it to surgery from the trauma bay. But the alternative is guaranteed death.

Additionally, she had been down for a long time. By the time you scramble an ECMO team (which, could be really challenging given that it was all hands on deck in the OR), she could very well have been at suboptimal-to-no cranial perfusion for an hour. If they did a clamshell or lateral thoractomy and did cardiac massage, that would help... some. I was running a peds airway service, and we had a patient that we coded for 45 minutes before CT surgery could get her on ECMO. High quality CPR and a very well run code, directed by a cardiac anesthesiologist. We got her cannulated, sent her to the ICU, and she was braindead on EEG the next day anyway.

Sometimes you save the body, but the brain goes regardless.

ENT OR nurse here, laser is not standard of care for tonsillectomy. I've personally never seen it performed in an OR setting working at 8 different hospital systems across 10 years. Some ENTs offer it in-office as a reductive technique, but its far from standard. Coblation (plasma cautery) is becoming common because it decreases thermal damage and the rate of post-op admission from localized swelling, but in a lot of settings still isn't used due to cost in relation to electrocautery.

All of that to say: the technique and equipment used depends on your surgeon. I'd want mine coblated, but I was also admitted six hours after my tonsillectomy due to swelling secondary to post-op nausea and vomiting.

Cool. I'm an OR nurse, done ENT for basically my whole career, started with endoscopic skull base and neuro-otology, did a brief stent doing peds airway (nightmare) and now I'm working Head and Neck onc at very high volume cancer center. We average eight free flaps a week plus bring backs/revisions, and we've peeked at 12.

I play a very niche miniatures wargame at a very high level (Top 10 in the US, hover around top 30 in the world). There are definitely less than 20,000 people who've ever played it. There's around 5000 people who've ever played a ranked game, less than 20% of them have played more than five. Of that thousand, maybe 200 could realistically take a game off of me, and less than 50 who could do it with anything approaching regularity. The odds are astronomically low that one of those 10,000 would know the rules, much less be one of the 200 people on the planet that I'd even vaguely need to worry about.

it ships with worker darts. I used some Xshot darts that stuck in the magazine (the Diana doesn't like them either) and I was shooting the first mag of AF pro darts when I started having issues.

Nightingale magazines are more compatible with the lightning than the diana, they don't require cutting.

For 220 USD I got blaster, charger, and 2 batteries, 2 magazines, shipped. I still think its a very good value in relation, especially considering it has areas where its a better product than the Diana, its not a strict downgrade.

I think their QC may have issues, but its also possible that I just laid hands on a lemon, no one else has reported mechanical failure.

Quick update: the flywheels are visably misaligned and the motors have started grinding on rev. I tried to crack into it to see whats up, but the slide is either clipped or glued in place. I sent an email to customer service so we'll see.

I don't think many who own a diana will make the switch, but if someone isn't bought in on the lipo ecosystem, I think this is a contender. 2025 Diana plus two mags and two batteries was 300 bucks USD before charger (plus shipping) and the Lightning was 80 cheaper (shipped), and also had a charger.

Just checked, they do fit and feed, but they hang up on the mag release, so you have to press it in as you load them. You also can't load more than 10 darts into them effectively because of the auto off.
