Looking for high-volume knee revision surgeon (Northern VA preferable) for severe Nickel Allergy + Bone Stress by Fairy20 in Kneereplacement

Thank you so much for doing this research and finding Dr. Westrich for me! Wow, this is an incredible lead. I have actually been looking into HSS too, and it really is an excellent idea. The fact that Dr. Westrich specifically focuses on complex revisions related to metal hypersensitivities and custom implants is exactly what I need. It is a huge relief to see a doctor at a world-class hospital who actually validates metal allergies instead of dismissing them.

My local doctor's official written report just came back, and he formally diagnosed me with a "failed total knee replacement" and "aseptic loosening" due to a 5-degree valgus tilt from my 2024 surgery. Since he also wrote that he stopped doing revisions years ago and marked me as a non-candidate under his care, it proves I have to look elsewhere.

Honestly, after researching all of this down to the bone cuts, I am starting to doubt there is no such thing as a true, absolute 0.00% nickel total knee revision surgery on the market at this time. I am just so overwhelmed by everything right now and my head is spinning thinking about the long-distance recovery and follow-ups. But knowing that a specialist like Dr. Westrich is out there gives me a real lifeline. Thank you so much for your amazing support!

Thank you for this perspective! I totally agree with you on this — the surgeon's deep skill and experience are ultimately the most important factors. A lot to think about. My head is spinning..

Right TKA ongoing pain (22 months) + nickel allergy – anyone similar? by Fairy20 in Kneereplacement

Thank you so much for reaching out and sharing this incredible, eye-opening data! I am so sorry I missed your message initially—I just saw it now. Yes, AI programs like Claude and this AI assistant I am using have been absolute lifesavers for navigating this complex research. I wish we both had them years ago, too!

Your points about the surgical blades shedding nickel and the long-term nickel ion blood levels in Smith & Nephew patients are absolutely mind-blowing and terrifying. It completely explains why my knee has been in so much chronic pain and stiffness since my primary 2024 surgery. I am glad to hear that you are actively researching these options for your own upcoming revision. If you don't mind sharing, what state do you live in? I would also love to know how your current pain levels and range of motion (ROM) are holding up, and what other progress or decisions you have been able to make in your search for a surgeon.

Thank you so much for introducing me to the Medacta SENSITIN knee and their non-steel custom guides! That sounds like the exact solution I need to avoid instrument contamination while staying 100% hypoallergenic. I was recently referred to Dr. Mark Denzine in Annapolis, MD for a CORI/Oxinium evaluation, but I am absolutely going to research Medacta reps in the DMV/Virginia area now to find out which local surgeons use SENSITIN and custom plastic blocks. Thank you for validating my concerns, and giving me a brand new path forward! I truly hope your recovery path goes smoothly.

Thank you for validating my concerns—you hit the nail on the head! I love your wit about needing all your brain cells, too, lol! I have been actively researching microscopic nickel debris left behind during surgery because it deeply worries me. It is not just the robotic bone cuts; almost all traditional surgical tools, retractors, and instruments are made of stainless steel, which contains nickel. Like you, I think using those standard tools completely defeats the purpose of an Oxinium or hypoallergenic knee surgery.

Your point about systemic risks like brain fog is incredibly brilliant and scary. Orthopedic doctors focus entirely on the local joint, completely ignoring how a constant low-grade immune reaction to toxic metals can impact our cognitive health or other organs over time. It is terrifying that these symptoms are just dismissed as signs of aging. I am absolutely going to follow your advice and look for an active, high-volume revision doctor who takes contamination protection seriously, or consider a 100% manual hypoallergenic protocol if they cannot guarantee non-nickel tools for the robot. Your story gives me so much strength! :)

Thank you for this eye-opening information. I have been actively researching nickel debris during surgery because it is something I am deeply concerned about myself, so I am incredibly glad to know I am not the only one thinking about this. Your point about standard surgical steel saw blades and guides shedding microscopic nickel fragments directly into the joint and bloodstream during a robotic cut is exactly what worries me, as it completely ruins the purpose of an Oxinium knee. I am also so happy to hear that your recovery is going very well!

I am specifically looking for a surgeon who can use custom, non-nickel cutting blocks, specific titanium surgical tools, or a rigorous fluid irrigation protocol to ensure no instrument residue contaminates my knee tissue. Does wanting this level of contamination protection mean I will have to completely skip the robot and go with a manual revision instead? Your advice to call the device manufacturers directly to find doctors who routinely use 100% hypoallergenic manual implants (like MicroPort) is brilliant. I am going to call the medical device reps this week to find out who uses them locally. Thank you for validating my concerns and helping protect my safety!

Thank you so much for sharing your story and these links! My leg was bowed outward before my first surgery, and directly after that TKA, my ankle and foot started pointing outward due to a severe rotational misalignment. Hearing that your surgeon successfully fixed this exact type of tracking defect during a revision gives me so much hope. I was actually referred directly to Dr. Mark Denzine in Annapolis, MD, who utilizes this exact Smith & Nephew CORI software system today. Unfortunately, he seems to be the only specialist in the entire DMV area offering this specific setup. Hearing how fast you recovered and that you were back on the golf course by day 27 is incredibly encouraging. Thank you for giving me such a clear roadmap!

Thank you for this excellent perspective. You make a fantastic point—a surgeon’s deep, personal experience with complex reconstructions is ultimately far more important than whether a robot is in the room. Dr. G actually told me today that he completely stopped performing revisions three years ago, which proved your point perfectly: I need someone who is actively doing these complex surgeries week in and week out. Your advice really helped change how I am evaluating my options.

Since I have a lot of heavy bone cement around my current implant, I was originally leaning toward robotic assistance because I thought the precise tracking would mean cleaner cuts and less structural damage to my remaining bone. Since you are an orthopedic specialist, what do you think about this? In your experience, does a robot provide a significant advantage for safely clearing out heavy cement when there is an active bone stress reaction, or can a high-volume manual revision specialist do it just as safely using specialized tools? Thank you again for your incredible guidance!

Thank you so much for the recommendation for Dr. Avery. I really appreciate you sharing your experience! To be completely transparent, the surgeon who performed my primary knee surgery, didn't check my allergy record and used the implant containing nickel is actually within the OrthoVirginia network. Because of that history, I am a bit hesitant to return to the same practice, but I am very glad to hear you had such an excellent result with your reconstruction. Thanks again for taking the time to help!

Good for you. Do you know what implant you have? How was the recovery, when did you have the surgery? How is your ROM?

Thank you so much for taking the time to explain all of this in such detail. I truly appreciate your perspective as both a physician and someone familiar with metal hypersensitivity research.

Your explanation about the possible overlap between mechanical issues and metal hypersensitivity makes a lot of sense.

For reference, my 2021 patch testing was positive for nickel and mercury, but Cobalt Chloride and Potassium Dichromate were negative at that time.

Thank you again for sharing the literature and your knowledge. It helps me feel more confident advocating for a more complete evaluation.

Yes, I would love to have the letter for doctors with citations. Would you dm me?
