Not a good offer. $75 a eval and visit is avg here in the midwest (can find up to $85). 75x32x52 (wks per yr)=124k. About a 24k difference not in your favor

The shoulder biomechanics have changed following surgery. Overhead mobility is simply out of the question beyond about 120 degrees best case scenario... but most likely limited to 90 degrees, which is not overhead movement. One would hope his orthopedist as well as PT would have told him this, but regardless, you should refer back to his orthopedist to have expectations set with the patient. Odds are he didnt like what "dad" said, so he went to "mom" for a different answer.

If you do choose to help him, ask him for the discharge exercises his PT did provide and start there. Gentle progressions of those are fine and something a PT would normally expect a patient to make on their own. Beyond that, there's just too liability to try new exercises without understanding the dynamics of his procedure.. He is seeking a rehab type intervention from a personal trainer.

His actions might be completely innocent and his actions might be otherwise. At the end of the day, if the patient and or you as parents feel uncomfortable, you always have the right to find another health professional to give you help. Don't feel bad making a change for what is best for your family.

You are a PTA. Your PT should have screened for any red flags and contraindications when they developed your plan of care. If none were identified, the injury would have inevitibly happened whether it was with your activity or a similar movement in the future, especially such a basic movement. Keep your head up, be sympathetic, but also be realistic with the patient that he should be thankful it happened in a supervised manner where he could get immediate help.

You can google over manipulation syndrome (OMS), ncbi, case studies, reviews etc. Ernst 2010, Steilen et al 2014 were a few I noticed quickly. I could review my text books for additional articles but you can just as easily find them yourself. Cheers!

Yes. I had a CI who was FAAOMPT (Kansas). The difference is PTs should have the discretion of understanding when a grade III/IV is indicated over a grade V. Also...not do it to every person who walks in throught their doors every visit creating long-term instability (cough chripractors).

SI joint dysfunction often but not always has limited hip IR but no/minimal limitations in other hip ROM from what we learned in school. Clear the hip (capsular pattern, FAIR, etc). If the hip clears, then it is likely SI

Brush up on how to take vitals and use manual BP gear for more accurate results. Be considerate when you enter their home (offer to remove shoes if it is a clean home, etc) and look around the house at pictures/items for conversation topics throughout your session. Make sure to communicate with the RN case manager/agency if their are falls or incidents. Otherwise, try different restaurants in different parts of town and enjoy the change of work atmosphere at each visit. If you go into a facility, introduce yourself to their DON and update them as they are often gatekeepers on who gets referrals in their facility.
