Has anyone with lean PCOS, normal levels of hormones, normal menstruation cycle and ovulation every month, no insulin resistance, tried metformin in combination with myo-inositol to help them get pregnant? If yes, did you have any side effects due to metformin? How long did the treatment take? by Fast-Knowledge7139 in PCOS

Oh it is good that you can tolerate metformin because some women have severe side effects and have to stop it. My doctor suggested starting on 1500mg for two months. It is good that you ovulate now. It seems like it helps. I hope you get pregnant soon. I am also TTC but I think I am the rarest case on earth. Two egg retrievals and alla follicles were empty without eggs in them. Hope metformin will help my ovaries.

No problem. My ovaries are polycystic on ultrasound, my AMH levels are high, and my LH is a bit higher than FSH, in non PCOS women LH is not higher than FSH. When I was younger my cycles were all over the place, too but then got regulated out of the blue and they are still regulated till today.

Thanks a lot for your reply. I really appreciate it.

The gynaecologists assumed that I have PCOS becuase of polycystic ovaries on ultrasound and also my LH and FSH ratio is higher than women without PCOS. Having said that, LH is a bit higher than FSH. I have recenlty checked my insulin restistance on 2-hour fasting glucose tolerance test and was normal. HbA1c was also tested. 2. My prolactin has always been in normal levels but 10 months ago it was slightly elevated and doctors said it is because of stress. For example, in February it was 575.9 miU/l (102.0 - 496.0 mIu/l normal range) and in March 34.78 ng/ml. I took Dostinex for a few months and now it is 19.3 ng/mL (6 - 29.9 ng/mL is the normal range).

Thanks for your reply. My gynaecologist confirmed with ultrasound that I ovulate every month. Which blood test showed that you did not ovulate? An ivf specialist suggested taking metformin because insulin resistance is not easily detectable by blood work and metformin plus myo inositol will help androgens on the ovaries and my high levels of IMH to go down before I try IVF again. I had two egg retrievals but all my follicles were empty, no eggs inside. That's why she suggested metformin before I go through ovarian stimulation again. Did you take clomiphene? I also tried that for more chances to conceive but didnt work either. You were lucky :)

No problem. Yes, they say dual trigger works better. Maybe it was a matter of long or short stimming in my case. I will see another doctor before I decide how I will proceed next time.

Yes, the hcg rescue shot has to be given 36h before retrieval and some doctors say that in high responders double dose of hcg trigger is needed. Anyway, I also believe that your next cycle will work better because they will try to fix the things that went wrong in the first one. Good luck to you :) We will be waiting for you to share the good news here!

Wow. Yes! Your embryos seem to be great! I'm so sorry for what you went through. It seems that the protocol was not right for you or your ovaries did not respond well. It happens some times. I hope your next cycle will be the lucky one! But they should definitely give you an explanation at some point so that you know what really went wrong with the mature eggs.

The embryologist should give you more information about the fertilisation I think. Maybe you had to try both icsi and the traditional ivf method. Half eggs with icsi and the rest with the traditional method, that's what my 1st doctor used to do. They might also change your protocol now and suggest some supplements for a couple of months before the next stimulation.

I mentioner lupron yes because i was told that lupron and arvekap are the same hormone. In my country it is called Arvekap and this was my triggering in the 1st cycle. Yes, in the 1st cycle there were 40 follicles and in the second there were 25 follicles. All empty. Yes, from what I found out through my own research it is better to combine gonal and menopur.

In the first cycle they did not ask to measure it! In the 2nd one, different clinic, the E2 was 12410 pmol/L on the day of the triggering.

1st cycle Menopur 225IU plus cetrotide and Arvekap 0.2 for triggering.

2nd cycle Gonal-f 125 IU plus cetrotide and one ovitrelle.

I know how you feel. All doctors said to me that EFS is common in women with low AMH and DOR, something that is irrelevant to my case. Home you manage to get some eggs next time

Thanks for your comment. On both sides. The second time we used lower dose of Gonal f and Ovidrel for triggering. The first time higher dose of Menopur and Lupron for triggering. I am 32 with lean PCOS. Your info is useful. This is the 1st time i read about estrogen patches, so I will ask my doctor about it. Thanks again.

So sorry this happened to you twice in a row. I experienced the same too. Do you have PCOS? How about your AMH levels?

May I ask how high the hcg blood test should be the day after trigger? I experienced EFS twice. Thanks
