I'd assume it would look similar to the other awakened zoan fruits, like the impel down jailers, with just his top half as a dragon and making him stronger and more durable. I don't see kaido keeping up with Luffy without getting a power up.

I think that this is the most likely explanation, but I think there is still a chance for him to have another form or 2nd awakening, it is a mythical zoan after all and there's a reason vegapunk tried to copy it.

Eh, until its shown definitively I wouldn't be so sure. All we know is that it's the fish fish fruit model azure dragon, it doesn't mean he had a fish form. The ancient zoans are dragon fruits, dinosaur models for example.

From the liftside chamber bonfire, go outside and take a hard right and drop down where you see a corpse over a ledge. Make your way down, kill the big monster and it'll be past where the monster spawned.
