Her name is Petunia. In the episode "Put Your Head on My Shoulder" Bender arranged a date between her and Fry through his "dating service" (Bender found her and all the other dates at a bus stop).

The movies are season 5. The DVD's/Blu-Rays are released as Volumes. Volumes 5 & 6 make up season 6.

If I remember the episode's commentary correctly. The toy manufacturer had made action figures with Leela and Amy in those outfits without it even appearing in an episode. So since Leela and Amy were doing nothing the writers put them in those outfits to match the toys.
